One of the most consequential medical disputes within the Cash–Landrum case was not whether Betty Cash lost hair, but what caused that hair loss. For supporters of the UFO-radiation interpretation, the photographs of Cash’s scalp became a visual symbol of severe exposure. For sceptics and several medical commentators, however, the diagnosis reportedly reached during her treatment pointed in a very different direction: alopecia areata, an autoimmune disorder that can produce dramatic patchy baldness without any radiation injury.
The importance of the diagnosis lies in its explanatory power. If Cash’s hair loss was consistent with alopecia areata and supported by pathology findings, then one of the case’s most widely cited physical effects becomes less persuasive as evidence of radiation exposure. If the diagnosis was mistaken, then critics may have dismissed a genuine injury. Alopecia areata is an autoimmune condition in which the body’s immune system attacks hair follicles, producing non-scarring hair loss. The classic presentation consists of smooth, sharply defined round or oval bald patches. How the Cash diagnosis was reportedly made
Accounts drawn from reviews of the Cash–Landrum medical record state that areas of alopecia were documented during Cash’s later hospitalisation rather than immediately after the incident. According to sceptical analyses of the case, a dermatologist reportedly described the affected areas as rounded patches with evidence of hair regrowth, features considered compatible with alopecia areata rather than widespread follicular destruction.
The most significant piece of evidence cited by proponents of the alopecia explanation is a scalp biopsy. Reviews of the case, particularly those discussing the hospital records, report that pathology findings were interpreted as supporting alopecia areata. In ordinary dermatological practice, biopsy can be used to distinguish among different causes of hair loss when clinical appearance alone is insufficient. Supporters of the Cash–Landrum radiation hypothesis have approached the evidence differently. They generally argue that focusing on alopecia areata isolates the hair-loss question from the wider constellation of symptoms Cash described. In this interpretation, scalp findings should be evaluated alongside reported skin injury, eye problems, gastrointestinal symptoms, weakness, and other complaints rather than treated as a standalone dermatological case.
A further point of contention concerns the pathology itself. Accounts of the dispute note that not all commentators accepted the biopsy interpretation. The reported challenge from radiation specialist Peter Rank illustrates how the disagreement extended beyond witness testimony and into competing readings of medical evidence. The argument therefore became not merely “alopecia versus radiation” but also “whose interpretation of the medical record is most persuasive”.
What the alopecia diagnosis can and cannot explain
The alopecia areata diagnosis addresses a specific question: why Cash developed visible bald patches. It does not automatically explain every symptom reported after the encounter.
From a medical perspective, alopecia areata is a recognised mechanism for patchy, non-scarring hair loss and can produce appearances that resemble a serious injury to observers unfamiliar with the condition. At the same time, establishing that a person has alopecia areata does not by itself determine the cause of all other reported medical problems. This is why the diagnosis remains a focal point rather than a complete resolution of the Cash–Landrum medical controversy. The key evidential value of the diagnosis lies in showing that there was a plausible, medically recognised, non-radiation explanation for one of the case’s most visually compelling symptoms.
In that sense, alopecia areata became one of the most important competing mechanisms in the Cash medical dispute. It transformed the discussion from a straightforward claim of radiation-induced hair loss into a narrower and more difficult question about diagnosis, pathology, and the interpretation of incomplete medical records.
